The Community Scholarship Program is active from November through June each school year. The information contained on this page is NOT to be considered current at any other time.
Before Getting Started Please read all of the information provided on the Scholarships and Criteria page before beginning the process of completing your application. To apply for a scholarship through the Community Scholarship Program, you must submit the scholarship application form and all supplemental documents as instructed. If you have questions about any part of the scholarship application process, please contact us.
Application Form You must check all scholarships for which you are applying. Only one Scholarship Application Form should be submitted, even if you are applying for multiple scholarships. The application form is designed to be completed on-screen and then submitted online.
The form cannot be saved for retrieval at a later time, so you should not begin entering data until you can devote sufficient time to completing the entire form in one sitting. Some applicants find it helpful to print a blank copy of the form to complete by hand as time permits, and then use this "rough draft" to make completing the form online faster and easier.
1. Certification Form The Community Scholarship Program Certification Form is designed to be printed, reviewed, and signed. You must have Adobe Reader to access this file (Adobe Reader is free and can be downloaded at www.adobe.com/downloads). The Certification Form must be mailed or delivered to the Community Foundation office. Copies of the form are available in your guidance office or it is provided below:
2. Essay Submit any required essay (as instructed on the Scholarships and Criteria page) by electronically attaching to the application form. Please make sure your name and the name(s) of the scholarship(s) for which you are applying are on each page of the document.
3. Letters of Recommendation
Submit any required letters of recommendation as instructed on the Scholarships and Criteria page.
4. FAFSA4caster Reports or FAFSA Student Aid Report
If a scholarship for which you are applying considers financial need in the selection process, the "Supplemental Requirements" section for that scholarship (as listed on the Scholarships and Criteria page) will indicate that you must complete the FAFSA4caster.
The FAFSA4caster is a tool designed by the United States Department of Education to provide an early estimation of a student's eligibility for financial aid. To access this tool, visit http://www.fafsa4caster.ed.gov/.
Follow the instructions for data entry as indicated on the FAFSA4caster website, print the FAFSA4caster Entry page (after entering your data and before clicking the Submit button) and the College Cost Worksheet page showing your calculated EFC (Expected Family Contribution) figure. Please use the "print preview" function prior to printing to ensure that all information contained in the documents is legible. You must write your name at the top of both documents and submit them as instructed below.
If you have already completed the FAFSA, you may submit your Student Aid Report (including the Expected Family Contribution number) in lieu of the FAFSA4caster reports.
5. Official Grade Transcript An official grade transcript is required and must be provided by your high school guidance counselor in addition to the Transcript Verification Form. Be sure that your PSAT, SAT, and/or ACT scores appear on your transcript. If they do not appear on your transcript, you must submit official documentation of your scores along with your transcript. Your official grade transcipt and transcipt verification form (see below), will be picked up by the Foundation Director directly from your school's guidance counselors.
6. Transcript Verification Form You must print the Transcript Verification Form and submit it to your high school guidance counselor prior to the application deadline. The counselor will complete the form, attach your official grade transcript, and prepare the information to be picked up by the Foundation.
HHHS Applicants: Please sign up in the guidance office so that Mr. Wilkerson and Mrs. Wilmes will know to have your transcript ready.
*****If you have applied for the Lilly Endowment Community Scholarship Program - you DO NOT need to request another transcript or Transcript Verification Form. We will already have that on file.*****
Compiling Supplemental Documents Assemble any documents listed as "Supplemental Requirements" for the scholarships listed on the Scholarships and Criteria page. Please note that not all supplemental requirements are the same. Include supplemental documents only when they are listed as a requirement.
Only one copy of the Certification Form (see below) is required. If you are applying for more than one scholarship AND the essay submission for each scholarship is the same, please only attach ONE ESSAY to your application.
Use paperclips or binderclips to assemble your supplemental documents and be sure that your name and the name of the scholarship are clearly indicated at the top of each page. Supplemental documents submitted in a report cover or binder will not be accepted.
Submitting the Application and Supplemental Documents Your application form must be submitted online by the application deadline. Mail or deliver your supplemental document(s) to arrive in (NOT POSTMARKED BY) the Community Foundation office by the application deadline. Required essays MUST BE submitted electronically with the application form; however, all other supplemental documents must be mailed or delivered as instructed. Applicants are responsible for complying with all Community Scholarship Program instructions and for ensuring that all information provided is legible.
Submission Deadlines Applications and supplemental documents are due on the date indicated for each scholarship listed on the Scholarships and Criteria page. If the date falls on a holiday or weekend, the application is due the preceding weekday.
